Saw-blade used for re-enforcing the roof.

Hockey puck Predator roof mount. (no longer made however)

Galaxy 93t, mounted to top of Dose, then L-brackets holding everything to floor. This is a old pic however. Radio and new Palstar meter is now mounted similar to way my radio is in my van, pictured below.

Jesse James, is that an amp in the floor? What kind? How many Watts?
As for the setup, I don't have any pics but in my 99 3500 pickup the roof is "double walled" about 3/16" - 1/4" between the two panels. Short...very short self tapping screws through the radio bracket into the roof did the trick. Re-installed the headliner, pushing the mount through cloth so you can mount the radio. Installed the "gear keeper" for the 'ole RK and we're in business

Drill a hole like JJD did and roof mount that Pred10k... That'll solve your "visible coax" issue - it'll be hidden in the headliner. I know he said the puck mount is no longer made, but I'm sure there's gotta be something out there similar, if not better.
Didn't know what kind of antenna you were running. thanks for cluing me in. I now understand why you specified needing a grommet and silicon to seal things up. Just don't go overboard on the silicon - a little goes a long way.

Hmmm.... sounds like your headliner is made of sheet metal rather than cloth. Can you drop down the inner layer? If so, then you should still be fine drilling through the outer layer and mounting like JJD. If you can't drop the inner layer, then I'm not sure how you'd want to do it.
You could cut an access panel in the inner layer (large enough to insert the saw blade and so you can bolt everything up)...making one end of the access panel a hinge (riveted to the original sheet metal) and the other a lock - for future access/mods. Another possible closure choice could be velcro (glued or riveted to each part of the access panel / lid). You might even consider searching for a cloth headliner.
Just my .02

well it does have a cloth head liner but what im thinkin is the guy i bought this truck from took the sun visors out and put a gun rack aka storage rack up there so im thinkin i may can drill my hole threw the top sheet and fish the coax threw the roof to one of the holes that is left in the roof from where the sun visors once where and it has plastic trim down the winshield frame and i could possibly tuck and hide the coax behind that and drop it in the dash to hide it there and i am mounting the radio beside my 4wd shifter on a pedastal the back of the radio will be almost against the fire wall leaving just enough room coax, powercord hookups and you won't even be able to see the coax at all except where it drops outa the roof threw the visor hole. Now whether i can do this the way i am thinkin is another story.
I wish I had a digital camera -- I'd take pics of my install and post 'em for you.
Based on your description, you should be able to run the coax down the A-pillar (I ran mine down the passenger side and into the dash - all hidden) without having to pull through the sun visor hole.
My headliner is cloth and is basically supported by the pillar plastics (interior lighting areas add support as well). You should be able to snake a coax line from rear-to-front of your cabin without too much difficulty - removing the owner-installed storage unit doesn't sound like it would be terribly difficult to do and would make for a cleaner install.
If you can take some pics and either email 'em to me or post 'em here, I might be able to give some better suggestions. It's been a long time since I've seen inside a 1991 model year vehicle (my dad had a Sonoma - cloth headliner).
